How to Submit a Takedown Notice

Counter-Notice Procedure
If you believe that your content was removed or disabled by mistake or misidentification, you may submit a counter-notice. Your counter-notice must include the following elements:
- Your physical or electronic signature.
- Identification of the material that has been removed or to which access has been disabled, and the location at which the material appeared before it was removed or disabled.
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of a mistake or misidentification of the material to be removed or disabled.
- Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal district court for the judicial district in which your address is located (or Davidson County, Tennessee if your address is outside of the United States), and that you will accept service of process from the person who provided the takedown notice or an agent of such person.

Repeat-Infringer Policy
fashionned.com has a policy of terminating the accounts of users who repeatedly infringe the copyrights of others. If a user is found to be a repeat infringer, we will take appropriate action, which may include disabling their account and prohibiting them from using our services in the future.
No Knowing Misrepresentations
Submitting a takedown notice or counter-notice with knowingly false information is a serious matter and may result in legal consequences. If you knowingly misrepresent that material or activity is infringing, or that material or activity was removed or disabled by mistake or misidentification, you may be liable for damages, including attorneys’ fees.
